Your Reconstruction for Stability

What Is Major Bone Grafting?

Severe bone loss from trauma, tumors, or congenital conditions requires extensive reconstruction. Standard grafting procedures cannot address these significant defects. Major bone grafting rebuilds large sections of missing jawbone, restoring both function and facial structure.
Major bone grafting reconstructs substantial jawbone defects using larger volumes of bone graft material, often harvested from your hip, tibia, or skull. These complex procedures repair defects caused by traumatic injuries, tumor removal, or developmental abnormalities. The grafted bone provides the structural framework necessary for dental rehabilitation and maintains proper facial dimensions.

Patients with extensive bone loss that affects large jaw sections or multiple tooth sites need this advanced treatment. The procedure often requires harvesting bone from another area of your body, as synthetic materials alone cannot address major defects. Minot Dental Partners in Minot, ND coordinates with expert surgical teams when major grafting requires hospital settings. Recovery involves several months of healing before the reconstructed bone can support dental implants or other restorations.

Rebuild Severe Bone Loss

Benefits of Major Bone Grafting

Major bone grafting reconstructs significant bone loss from trauma, disease, or long-term tooth absence. It restores jaw structure for complex implant cases, repairs defects that affect facial appearance, and creates a foundation when standard grafting isn’t sufficient. You regain options for full mouth rehabilitation that seemed impossible before.

Solves Complex Dental Problems

You can address severe bone loss that prevents conventional implant placement. Major grafting rebuilds substantial areas, making full arch restorations or multiple implants possible when other solutions have failed.

Restores Facial Symmetry

Large bone defects create visible facial irregularities and asymmetry. Major grafting rebuilds your jaw to its proper dimensions, improving your profile and eliminating the hollow or collapsed appearance caused by extensive bone loss.

Uses Your Own Bone When Needed

For large grafts, bone can be harvested from your hip or jaw for the strongest, most reliable results. Your own bone integrates perfectly with existing bone, creating a solid, permanent foundation without rejection concerns.

Supports Full Mouth Reconstruction

You can receive comprehensive dental rehabilitation even after severe trauma or advanced gum disease. Major grafting provides the structural base needed for All-On-X implants or multiple individual implants throughout your mouth.

Prevents Further Complications

Extensive bone loss weakens your jaw and can affect your bite, speech, and ability to wear dentures comfortably. Major grafting stops this progression, protecting your remaining oral structures and improving overall function.

Permanent Structural Repair

Once healed, major bone grafts become living bone that lasts your lifetime. You won’t need repeated grafting procedures, and the reconstructed area supports implants with the same strength as your natural jawbone.

Extensive Jaw Reconstruction

Who Is Major Bone Grafting For?

Severe bone loss from trauma, disease, or long-term tooth absence requires more extensive reconstruction than standard grafting. Major bone grafting rebuilds significant portions of the jaw, restoring both function and facial structure for complex implant cases or overall oral health.

Significant Trauma or Injury

Accidents, fractures, or surgical removal of jaw portions due to disease have left large areas without adequate bone structure for normal function or tooth replacement.

Multiple Missing Teeth for Many Years

Losing several teeth decades ago has caused extensive bone deterioration across wide areas of your jaw, requiring substantial rebuilding before any restorative work can succeed.

Failed Previous Implants or Dentures

Years of wearing poorly fitting dentures or experiencing implant failures have accelerated bone loss, and now you need major reconstruction to move forward with treatment.

Do Any of These Situations Sound Familiar to You?

These serious concerns often indicate that major bone grafting is necessary to restore your oral health and quality of life.

Extensive Jaw Reconstruction Process

Major Bone Grafting for Complex Cases

Major bone grafting rebuilds significant areas of lost jawbone caused by trauma, disease, or long-term tooth loss. At Minot Dental Partners, we perform advanced grafting procedures to restore both function and facial structure.
1

Comprehensive Surgical Planning

We conduct thorough 3D imaging and clinical examinations to map the extent of bone loss and plan the reconstruction approach. For major grafting, we often harvest bone from your hip, chin, or other donor sites in your body to ensure the strongest possible graft. We coordinate with your medical history and may work in a hospital setting if the procedure requires more extensive surgical support.
2

Surgical Bone Reconstruction

We place large volumes of bone graft material to rebuild missing sections of your jaw, carefully shaping and securing the graft to restore proper contours. The procedure may involve plates, screws, or membranes to hold the graft in position during healing. Depending on the complexity, we may perform the surgery in stages to achieve the best outcome and ensure your comfort throughout.
3

Extended Healing and Rehabilitation

Major bone grafts require longer healing periods, typically six months to a year, as your body generates substantial new bone tissue. We monitor your progress closely with regular appointments and imaging to track bone formation and integration. Once fully healed, the reconstructed jaw provides a stable foundation for dental implants and restores proper facial support and appearance.

Frequently Asked Questions

Major Bone Grafting FAQs

Major bone grafting reconstructs significant bone loss in your jaw. These questions address when extensive grafting is necessary and what the comprehensive process involves for complex cases.

What qualifies as major bone grafting?

Major bone grafting involves rebuilding large areas of missing or deficient jawbone, typically caused by extensive tooth loss, severe gum disease, trauma, tumor removal, or congenital defects. Unlike minor grafts that prepare a single implant site, major grafting reconstructs substantial portions of your jaw structure. This may involve harvesting bone from your hip, shin, or skull in addition to using processed donor bone or synthetic materials. Major grafts often span multiple teeth areas and require more complex surgical techniques. Dr. Rustemier’s focused training in periodontics and extensive experience with reconstructive procedures make Minot Dental Partners equipped to handle even the most challenging bone grafting cases.
Several situations require extensive bone reconstruction. Long-term tooth loss causes progressive bone deterioration that simple grafts cannot address. Advanced periodontal disease may have destroyed significant bone around multiple teeth. Facial trauma from accidents can fracture or damage large sections of jawbone. Tumor removal sometimes requires taking bone, which must be rebuilt. Some people have developmental conditions affecting jaw formation. Additionally, failed dental work or infections can compromise bone structure. Major grafting restores your facial anatomy, creates a foundation for dental implants or dentures, and prevents further bone loss that would affect your appearance and function.
Major reconstructive grafts often use your own bone, called autogenous grafts, harvested from your hip (iliac crest), shin (tibia), or skull. Your own bone is the gold standard because it contains living cells that promote optimal healing. For less extensive major grafts, processed donor bone (allograft) or synthetic materials may suffice. Often we use combinations, mixing autogenous bone with other materials to extend the graft volume. The harvest site depends on how much bone is needed. Dr. Rustemier will explain which approach best serves your reconstruction goals and discuss what to expect from the harvest procedure during your consultation.
Recovery from major grafting is more involved than minor procedures. Expect significant swelling and discomfort for the first week, managed with prescribed pain medication. If bone was harvested from your hip or shin, you’ll have soreness at that site as well, potentially requiring crutches temporarily. Most patients need one to two weeks off work. Stick to a soft or liquid diet initially, gradually advancing textures as healing allows. Avoid strenuous activity for several weeks. Complete integration takes four to nine months depending on graft size. We’ll schedule regular follow-up appointments to monitor healing and ensure proper bone formation throughout your recovery.
Major bone grafts require substantially longer healing than minor grafts, typically six to nine months for complete integration. Larger reconstructions can take up to a year before implants can be placed. This extended timeline is necessary because your body must gradually replace the graft material with living bone strong enough to support dental implants. Initial soft tissue healing occurs within weeks, but bone remodeling continues for many months. While the wait seems lengthy, proper healing is absolutely essential for long-term success. Dr. Rustemier monitors your progress through periodic appointments and imaging at Minot Dental Partners, ensuring bone develops adequately before proceeding with implants.
Some major grafting procedures, particularly those requiring bone harvest from your hip or shin, may be performed in an outpatient surgical center or hospital operating room. This provides access to general anesthesia and additional medical support for more extensive surgeries. Less complex major grafts can often be performed in our office at Minot Dental Partners using IV sedation administered by a Certified Registered Nurse Anesthetist. The setting depends on the extent of reconstruction needed, which harvest site is used, and your overall health status. Dr. Rustemier will determine the most appropriate location for your specific procedure and coordinate all necessary arrangements.
Major grafting carries more risks than minor procedures, though complications remain relatively uncommon with experienced surgeons. Possible risks include infection, graft failure, nerve injury, excessive bleeding, or pain at the harvest site. If your own bone is used, the donor site can experience discomfort, infection, or rarely fracture. Graft material may not integrate as expected, requiring a second procedure. Smoking, diabetes, and certain medications increase complication risks. Dr. Rustemier’s focused training and meticulous surgical technique minimize these risks. Following all pre-operative and post-operative instructions carefully significantly improves your chances of uncomplicated healing and successful reconstruction.
Major bone grafting has good success rates, generally 85 to 95%, depending on the complexity of reconstruction, graft materials used, and patient factors. Success is lower than minor grafting because these procedures are more extensive and face greater challenges. Your own bone typically produces the most predictable results. Factors affecting success include your age, overall health, smoking status, medications, and how well you follow post-operative instructions. Dr. Rustemier’sr’s expertise in complex reconstructive grafting and use of advanced Cone Beam CT imaging for precise planning maximize your chances of successful bone regeneration, even in challenging cases requiring extensive reconstruction.
Major bone grafting is significantly more expensive than minor grafting due to the procedure’s complexity, additional materials needed, surgical time required, and potentially hospital or surgery center fees. If bone must be harvested from your hip or shin, this adds to costs. Pricing varies widely based on reconstruction extent and specific techniques required. While expensive, major grafting is often essential for restoring jaw function and preparing for implants. Many dental insurance plans provide partial coverage when reconstruction is medically necessary. Minot Dental Partners works with multiple financing companies to create manageable payment plans. We’ll provide detailed cost estimates during your consultation.
Occasionally major grafts don’t integrate as expected, though this is uncommon when procedures are performed carefully and instructions followed. If a graft fails partially, sometimes the remaining integrated bone is sufficient for modified implant placement. Complete failure typically requires waiting several months, then attempting the graft again with modifications to technique or materials. Second attempts often succeed because we can address factors that affected initial healing. While graft failure is frustrating and extends treatment time, most patients ultimately achieve successful reconstruction. Dr. Rustemier’sr’s experience with complex cases means he can adapt treatment plans effectively if unexpected challenges arise.

Dr.Sammy is amazing !! So kind and gentle – you can tell he truly cares and also loves what he does . He is very knowledgable , very good at explaining things . Very thorough .. weighs out pros and cons to help you make an informed decision . Does not push for unnecessary procedures . Looks at the whole picture first before jumping to decisions .. I wanted to switch to Minot dental partners from my prior dental provider because I’ve heard such great things about MDP and when I called to inquire about establishing care they were able to schedule me in the same week!! No months and months of waiting like other places. Dr. Sammy was so nice and so understanding about all of my concerns— great listener , very validating ! The staff are all so nice and you can tell they genuinely enjoy working there and working with eachother .. everyone makes sure your comfortable and feel welcome I also really really appreciate the fact they submit to your insurance first to determine what you may owe and then send you a bill at a later time as applicable rather than making you pay all upfront using an “estimated total” and submitting after you’ve paid Minot dental partners are amazing , I would 10/10 highly recommend .

Awesome experience all the way through. Friendly staff, nice facility, and an inviting atmosphere, would definitely take the edge off of visiting the dentist. I called morning with a massive cyst underlying two of my lower teeth. Originally I was provided a Tuesday appointment, but when I said that I was in severe pain and hadnt slept for a couple days, and asked if I could at least get a script for painkillers and or antibiotics, Sylvia asked me to hold on for a minute. A couple of minutes later she returned to say that one of the partners would come in to the practice to see me at 12:00. And the frank, no nonsense approach, some great surgery and clear options and post-op instructions just kept getting better and better! And you could tell that this was not a once-off event, the crew knew what they were expected to do, what their values are … This is not only good dentistry, but great patient-focus generated by even better leadership. Thank you. You have probably added at least five years to my life.

Sarah, my dental hygienist did such a great job! Through the years, I’ve had many hygienists worked on my teeth and Sarah was my lasted one and she did magic! My teeth felt so clean without unnecessary pain. She worked on my ugly stains and managed to lift all of them off. I was super impressed and couldn’t be any happier. Sometimes I’d felt that the hygienist did not much and not worth the time and money. But not with Sarah. I’ll go back to her for sure.
